Campaign Not Saving - Xbox One

I bought the game (digital download) a few days ago.  Everyday day now I beat the first 3 or 4 missions and the next time I come back to play my saves are lost!  It has happened 3 times now.  I have tried the following:

1. Powered down the Xbox One and unplugged for 2 minutes. 
2. Tried deleting my Xbox Live profile and re-adding;
3. I make sure I am signed in with my Xbox Live profile before I launch game.
4. I make sure I select Save and Quit every time I stop playing. 

Still it does not work.  This happens without even turning off the Xbox One.  As soon as I close the game and come back my Campaign saves are gone.  I have even seen when I select "Save and Quit" and get back to the main menu I am not able to select "Resume Game", it is greyed out with a lock next to.

Please help!!
