I would recommend setting up a Static IP address as this will help to create stability in you connection as well as prevent you from having to reconfigure your port in the future. When you set up port forwarding for one ip address, other ip's will have some trouble connecting to those ports. For example if you set up port forwarding on your Xbox 360, your PS4 may encounter issues connecting. Also, if you port forwarded to your Xbox 360, and your connection was reset the IP address may not be the same for your Xbox 360. You can try setting up Static IP addresses for both of your consoles, set one in DMZ and port forward the other using the following ports:
PSN Ports:
80, 443, 465, 983, 1935, 3478, 3479, 3480, 5223, 10070-10080 TCP
3478-3479, 3658, 10070 UDP
Xbox Ports:
53, 80, 3074 TCP
53, 88, 500, 3074, 3544, 4500 UDP
Call of Duty Ghosts Console Ports:
80, 81,443, 3074, 5223 TCP
36081, 3074, 3075, 3478 UDP
You can also try this method for setting up your connection on your PS4:
1. In the PSN Menu, go to “Settings”.
2. Go to “Network”
3. Go to “Set Up Internet Connection”
4. When it asks, “How do you want to connect to the network?” , go to “Use Wi-Fi “ or “Use a LAN Cable” (it doesn’t matter)
5. When it asks “How do you want to set up the internet connection?”, go to “Custom”
6. From here, set up your IP Address Settings, DHCP Host Name, and DNS Settings however you want.
7. ***Here’s the important part:***
When you get to MTU Settings, choose “Manual”
Set that to 1480.
8. Set up your Proxy Server however you want.
Test your internet connection.
